LOCKER RENTALS NOTICE

LOCKERS ARE AVAILABLE FOR RENTAL FOR
BDS$30.00 (A, B, C & NUMBERED LOCKERS)
BDS$20.00 (D LOCKERS)
& BDS$40.00 (BLUE LOCKERS)
FOR THE ACADEMIC YEAR 2021/2022
HOURS OF RENTAL ARE:
MONDAY TO FRIDAY
FROM 10:00 A.M. TO 12:00 NOON
&
2:00 P.M. TO 4:00 P.M.
IN THE FST FACULTY OFFICE.
NOTE:
FST Students who wish to rent a locker from the FST Faculty Office can complete the necessary *locker rental form here, and then visit the Faculty's Main Office from Monday, August 30, 2021 to complete the locker rental process between the hours and days provided above.
*Note that the above form provided is a fillable pdf form, so you can input the requested data/information directly on the form and then email the completed form afterwards to
fst@cavehill.uwi.edu and
claire.cadogan@cavehill.uwi.edu to be processed by the FST Faculty Office Personnel before visiting the Faculty Office to make the necessary payments.
N.B.: Students are only required to complete the section on the locker rental form where they will include their name, address, telephone number(s), email address, your signature and the date you completed the form. Please DO NOT fill in the "FOR OFFICIAL USE ONLY" Section on the form. Your completed form can then be saved and emailed to our FST Faculty Office Personnel as noted above.
Please be reminded also that
all persons visiting the FST Faculty Office MUST wear a face mask, sanitize their hands and adhere to the physical distancing rules.
